Conference registration

Registration opens: July 20, 2006
Registration is required for conference attendance. Please see information
below on how to register for the conference.

To
request a registration form:
Send e-mail to office@creative-arts-therapy.org
with "Registration form request" in the subject line
and your name, address, and phone number in the message line. We will
begin sending out registration forms from July 20.

About
the registration process:
Once we receive your request, we will send you the registration
form via e-mail as an attachment. Please fill-out the form and return.
Reservations for the get-acquainted party and pre-conference
reception party will also be included in the registration form.

About
payment of conference fees:
For participants residing in Japan, payment method will be
through bank transfer.
For overseas participants, we request payment through PayPal.
Details/instructions for payment will be e-mailed once we receive
your registration form. Please make payments for your conference fee
and get-acquainted party (if attending) within 10 days of receiving
instructions.
If you are also signing-up for workshops during the conference, then
please make all necessary payments once we inform you of which workshops
you were able to sign-up for.

Workshop
reservations:
All workshops require pre-registration. Fill-out the workshop
registration form included with the conference registration and return.(
Note: Workshop-only registrations will not be accepted. You must register
for the conference in order to sign-up for workshops.)
The initial closing-date for workshop registration will be August
20. If there is an overflow of reservations for workshops, we will
hold a lottery in order to decide which choices you have qualified
for. We will notify you by the end of August about the results, including
information on payment method. Please make payments for the conference
fee, workshops, and get-acquainted party (if attending) within 10
days of receiving the notice.

About
workshop selection:
Refer to the conference website for information on workshop
choices. Lectures, presentations, and a number of workshops will be
held concurrently, so please closely examine the program schedule
when making your selections.

Cancellation
Policy:
If you are unable to attend a workshop due to cancellation
or changes in workshop schedule, your fees will be refunded.
If for some reason you must cancel registration for the conference,
workshops, or get-acquainted party due to personal reasons, no refunds
will be available.

Additional
information:
Registration for the conference, get-acquainted party, and
pre-conference reception party will be accepted past August 20th,
until filled. Workshop reservation availability will be updated on
the conference website.
ATTENTION: REGISTRATION FOR THE CONFERENCE WILL
MEAN THAT YOU HAVE READ AND CONSENT TO THE FOLLOWING ITEMS:

About
video taping, audio recording, photography, and copying:
1.Visual and audio recordings, and also photography of conference
lectures, presentations, workshops, and poster sessions will be prohibited.
2.Abstracts and presenter information on the conference website
is copyrighted. Making copies without permission is prohibited.
3. The conference organizing committee may record a portion
of the conference presentation, with the presentersÕ consent for record-keeping
purposes or to post reports on the conference website. Participants
will not be identifiable in any recordings. The conference PR staff
will have an armband so that they can be identified.

Workshops:
Purpose: Workshops are intended for participants to learn
through experiential exercises. Observing workshops without participating
in exercises will not be accepted.
Language: All workshops will have either Japanese or English
interpretation, whichever is necessary.
Preparation: Unless otherwise stated, there is nothing participants
must bring. Typically for workshops, participants will either move
around or use media, so it is recommended that you wear comfortable
clothing that can be easily washed, if you get any stains on it. For
dance/movement workshops, you may want to bring a change of clothes
and towel.
Facility:
1. Workshop rooms require that everyone entering the room
to take off their shoes. If you need slippers, please bring your own.
2. Entering and leaving workshops once they have begun will
not be permitted.
3. Eating and drinking in the workshop rooms is not permitted.
4. We cannot be responsible for personal belongings placed
within the conference facility. Participants are responsible for monitoring
personal belongings.
*Note:
- The workshops are not intended for the purpose of providing
counseling or therapy. If you are in need of such, please choose workshops
accordingly, as they will require active participation.
- Please understand that workshops will be conducted in groups and
workshop presenters will not be able to attend to individual needs.
- Should any problems arise between participants, the conference organizers
cannot be responsible.
